Do you qualify or know someone who may? Submit a referral to PACE. To qualify the participant must meet the following criteria:
- At least 55 years old
- Are qualified by the state as being eligible for nursing facility level of care
- Are able to live safely at home and in the community
- Are living in the CV PACE service area
