Medical & Dental
Central Valley PACE provides comprehensive medical and dental care, including primary care, preventive dental services, routine checkups, and medication management. Our coordinated care team helps participants stay healthy, maintain their independence, and enjoy a better quality of life.
Therapy
Our licensed therapy team helps older adults improve strength, balance, mobility, and independence so they can continue living safely at home. Personalized treatment plans may include physical, occupational, and speech therapy, along with fall prevention and mobility training.
Home Care
Central Valley PACE provides personalized home care services to help older adults remain safe and independent at home. Our team may assist with daily activities and recommend home safety equipment, including ramps, handrails, and shower seats, based on each participant's care plan.
Meals & Nutrition
Central Valley PACE provides nutritious meals and personalized nutrition services to help older adults stay healthy and independent. Participants enjoy hot meals in a social setting, along with support from a registered dietitian, nutrition education, meal planning, and guidance to address food insecurity.
Transport Services
Central Valley PACE offers free, door-to-door transportation for medically necessary appointments within our service area. We coordinate rides to the PACE Day Center and specialty appointments using accessible vehicles to help participants get the care they need safely and conveniently.
Senior Day Center
Central Valley PACE Day Center provides a welcoming environment where participants enjoy nutritious meals, engaging activities, exercise, and meaningful social connections. Our daily programs help older adults stay active, independent, and connected while continuing to live safely at home.
