Can I continue living at home?
Yes. One of the primary goals of Central Valley PACE is to help participants remain safely and independently in their own homes for as long as possible.
Can I keep my primary care doctor?
PACE participants receive their primary care through the Central Valley PACE interdisciplinary care team. If you currently see specialists, your care team will review your medical needs and coordinate specialty care as appropriate.
Is transportation included?
Yes. Transportation is provided for medically necessary appointments, visits to the PACE Center, and other approved services included in your care plan.
Does Central Valley PACE provide care at home?
Yes. Depending on your care plan, services may include assistance with bathing, dressing, meal preparation, light housekeeping, medication reminders, skilled nursing, wound care, and other support services provided in your home.
How much does Central Valley PACE cost?
Costs depend on your Medicare, Medi-Cal, or other insurance coverage. Many participants who qualify for both Medicare and Medi-Cal have little to no out-of-pocket costs. Our Enrollment Specialists can explain your coverage and answer questions about costs.
Can my family or caregiver stay involved?
Absolutely. Family members and caregivers are an important part of the care team. We work closely with loved ones to coordinate care, communicate updates, and provide support throughout the participant's journey.
What happens during my first 30 days?
During your first 30 days, you'll meet with members of your interdisciplinary care team who will complete comprehensive assessments of your medical, therapy, nutritional, and home care needs. Together, they will develop a personalized care plan designed to help you achieve your health goals.
What makes Central Valley PACE different?
Unlike traditional healthcare, Central Valley PACE brings together doctors, nurses, therapists, social workers, dietitians, transportation, and in-home support into one coordinated program. This team approach helps participants receive the care they need while remaining independent at home.
Can I schedule a tour?
Yes! We encourage prospective participants, family members, and caregivers to visit one of our PACE Centers. Scheduling a tour is a great way to meet our team, see the center, and learn how Central Valley PACE can support you or your loved one.
