The PACE Model of CARE
PACE (Program of All-Inclusive Care for the Elderly) is a health plan AND a primary care provider. By delivering both services, PACE can better manage the whole person across the continuum of health care services. In order to achieve this, PACE partners with specialists, home health providers, hospitals, skilled nursing facilities and other community based services. Through these partnerships, PACE can care for the whole person and support them where they are – in their own homes and communities.
