PACE ADULT GERONTOLOGY NURSE PRACTITIONER (Merced PACE Center)
The PACE Adult-Gerontology Nurse Practitioner (AGNP) provides high-quality, compassionate medical care to participants of the Golden Valley Health Centers (GVHC) PACE program. This position works under the direct supervision of the PACE Medical Director and/or designated primary care physicians and is responsible for assessments, treatment planning, coordination, and delivery of care in the clinic, day center, homes, and residential settings.
Location and Schedule: This position will work at our Central Valley Merced PACE Center, scheduled Monday – Friday, 8:00am-5:00pm. Flexibility to work scheduled evening clinic hours, and when appropriate, act independently as a solo provider of services under telephone communication with a physician supervisor.
Compensation: $164,000 – $194,000 Annual Salary; Sign On Bonus and Relocation Assistance Eligible

PACE ACTIVITIES COORDINATOR (MODESTO)
Responsible for coordination of Recreation/Activities team, developing and facilitating recreational groups and programs which meet the social, emotional, cognitive, and physical needs of the participants, both individually and collectively.
Schedule: Monday – Friday, 8:00am – 5:00pm
Compensation: $25.22 – $26.48 an hour
Location: Will work in our Central Valley PACE Center in Modesto majority of the time, but will also assist at PACE Merced Center and Stockton PACE once it opens.
PACE CERTIFIED OCCUPATIONAL THERAPY ASSISTANT (MODESTO)
The Certified Occupational Therapy Assistant (COTA) is responsible under the supervision of the Occupation Therapist for implementation of a program of occupational therapy for PACE patients to help them to remain independent and live safely in the community.
Will work in our Central Valley PACE Center in Modesto majority of the time, but will also assist at PACE Merced Center and Modesto McHenry Outpatient Clinic.
Schedule is Monday – Friday, 8:00am – 5:00pm.

PACE CLINICAL THERAPIST (MERCED/MODESTO)
Under the supervision of the Medical Director, the Clinical Therapist is responsible for developing and implementing personalized care plans to ensure high-quality, team-based behavioral health services for participants. Key responsibilities include conducting initial assessments, providing clinical interventions, treatment recommendations, and coordinating referrals for participants and their families. This role requires physical presence at a Central Valley PACE center and the flexibility to travel for participant visits as needed. This role collaborates with a multidisciplinary team to provide evaluation, individual, family, and group therapy, as well as case management for older adults with mental health and behavioral needs.
Compensation: $94,000.00 – $100,500.00 Salary
PACE COMPLIANCE AND SAFETY SPECIALIST (MODESTO)
Under the supervision of the PACE Associate Director of Quality, Compliance, and Safety, the PACE Compliance and Safety Specialist is responsible for ensuring the safety and regulatory compliance of the PACE program. This role involves monitoring program adherence to Centers for Medicare & Medicaid Services (CMS) regulations, identifying and mitigating safety risks, and supporting continuous improvement efforts across the organization. The Compliance and Safety Specialist also develops, implements, and monitors safety protocols to protect participants, staff, and the overall program environment.
Schedule is Monday – Friday, 8:00am – 5:00pm.
Compensation: $26.87 – $28.21 an hour
PACE DAY CENTER SUPERVISOR LVN (MODESTO)
Has the responsibility for providing direct and indirect nursing care to assigned patients. Manages delivery of participants Activities of Daily Living (ADL’s) assistance in the Day Center. Supervises personnel care attendants (PCAs). Performs clinical tasks within scope of responsibility, under the supervision of the Director of Center Operations RN.
Will work in our Central Valley PACE Center in Modesto.
Schedule is Monday – Friday, 8:00am – 5:00pm with flexibility to work overtime.
Compensation: $33.79 – $35.48 an hour
PACE HEALTH PLAN MANAGER (MODESTO)
Responsible for providing oversight and coordinating the daily operations of health plan objectives including contracts, compliance, credentialing, communication, authorizations/claims, and disputes in a manner that meets established turnaround, quality and production goals, and regulatory standards.

PACE HEALTH SPECIALIST (CERES)
The Central Valley PACE Health Plan Specialist supports the team through data analysis, interpretation, reporting, and modeling health plan operations. Supports aspects of monthly and ad hoc reporting and assists in the preparation of pro form as based on historical, comparative, and projection data. Assists to ensure health plan operations, processes, procedures, policies, and compliance efforts are in compliance with regulatory agencies.

PACE HP – PROVIDER RELATIONS SPECIALIST (CERES)
The Provider Relations Specialist will serve as a key liaison between the organization and healthcare providers within the PACE (Program of All-Inclusive Care for the Elderly) model. This role is responsible for fostering and maintaining strong, positive relationships with providers, ensuring seamless care coordination, and supporting the organization’s mission to deliver high- quality, integrated healthcare services for seniors.

PACE MEDICAL TRANSPORTER – PER DIEM (MODESTO)
Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.

Schedule: Monday – Sunday – Per Diem (as needed). This position requires availability between the hours of 6:00am-6:00pm. Work hours will vary based on need, with shifts ranging from 2 to 8 hours per day when called upon, not exceeding 15 hours per week.
Compensation: $21.79 – $22.88 an hour
PACE OUTREACH SPECIALIST (STOCKTON)
The Outreach Specialist plans, organizes, and directs all outreach and marketing efforts. Involved in the intake and enrollment, and coordination with the interdisciplinary team to facilitate new PACE participant enrollment.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.

PACE QUALITY SPECIALIST (MODESTO)
Under the supervision of the PACE Quality and Compliance Coordinator, the Quality Specialist works to support the annual quality and Compliance (Q&C) work plans and special initiatives by monitoring, gathering, and preparing Q&C data for use by the PACE Interdisciplinary Team and Administration. The Q&C Specialist is a key position that works with all PACE departments to monitor and identify potential Q&C issues, monitor Q&C improvement initiatives, and prepare Q&C summary reports.
