PACE ADULT GERONTOLOGY NURSE PRACTITIONER
Location: Modesto

The Nurse Practitioner has the responsibility for providing direct and indirect care to the Participants in the PACE Health Clinic and Day Center. Works under the direct supervision of the PACE Medical Director and/or designated primary care physicians as described in the Golden Valley Health Centers PACE Nurse Practitioner’s Supervision Agreement, Golden Valley Health Centers Principles of Practice, and the California Administrative Code, Section 13799.541. The PACE Nurse Practitioner may be required to work scheduled evening clinic hours, and when appropriate, act independently as a solo provider of services under telephone communication with a physician supervisor.

PACE INTERNAL MEDICINE PHYSICIAN 
Location: Modesto
The PACE Internal Medicine Physician is responsible for providing quality comprehensive health care services to the geriatric PACE participants. This involves inclusion if an interdisciplinary collaboration of care within the PACE program. Focused on the improvement health status of patients, optimal provider productivity, and a safe, culturally sensitive, appropriate environment of care.

Responsible for developing and facilitating recreational groups and programs which meet the social, emotional, cognitive, and physical needs of the participants, both individually and collectively.  

This position works at our Central Valley PACE Center in Merced, CA.

Schedule is Monday – Friday, 8:00am – 5:00pm

Compensation:

$19.76 – $20.75 an hour

PACE ASSISTANT TRANSPORTATION COORDINATOR (MODESTO)

Under the general supervision of the PACE Transportation Coordinator,  the PACE Assistant Transportation Coordinator supports various aspects of the day center transportation services by assisting with planning and organizing the participant flow. 

Schedule is Monday – Friday, working from 8:00am – 5:00pm

Compensation:

$27.80 to $29.19 an hour

PACE CERTIFIED OCCUPATIONAL THERAPY ASSISTANT (MERCED)

The Certified Occupational Therapy Assistant (COTA) is responsible under the supervision of the Occupation Therapist for implementation of a program of occupational therapy for PACE patients to help them to remain independent and live safely in the community.

Compensation:

$35.48 – $37.25 an hour

PACE DIRECTOR OF CENTER OPERATIONS, RN  (MERCED)

The Central Valley PACE (Program of All-Inclusive Care for the Elderly) Director of Center Operations (DCO) is responsible for managing all operational aspects for one regional PACE center and associated Alternative Care Site’s (ACS’s) served by multiple PACE Interdisciplinary Teams (IDT’s)  to achieve optimum access to services, excellent customer service, quality, cost, and delivery performance in line with the PACE Mission, strategic goals and values, and serves as the liaison between Center, Support Service Management and leadership.

Schedule is Monday – Friday, working from 8:00am – 5:00pm.

Compensation:

$126,214.40 – $132,516.80 Salary

PACE ENROLLMENT SPECIALIST (MODESTO)

The Intake Specialist oversees the intake and enrollment process of new Program of All-Inclusive Care for the Elderly (PACE) participants. Responsible for educating new potential participants and their caregivers on the benefits of Central Valley PACE Program. Will work independently to gather information, set appointments for assessments and document skilled needs for the incoming cases. Interacts with family caregivers and participants, providing a tour through PACE center, gathering crucial medical and social needs for new participant for enrollment processes.

Schedule availability is Monday – Friday, 8:00am – 5:00pm. 

Compensation:

$25.22 – $26.48 an hour

PACE GERIATRIC SERVICES SPECIALIST (MODESTO)

Responsible for providing support and advocacy services to patients of the Central Valley PACE program, including participant screening, assessment, planning, coordination of patient services and care, counseling and referral.

Schedule is Monday – Friday, working 8:00am – 5:00pm.

Compensation:

$32.18 – $33.79 an hour

PACE GERIATRIC SERVICES SUPERVISOR  (MERCED)

Under the direct supervision of the PACE Clinical Manager, the Geriatric Services Supervisor is responsible for coordinating, supervising and evaluating the implementation of psychosocial support services to participants of the Central Valley PACE program including participant screening, assessment, planning, coordination of patient services and care, counseling and referral services carried out by the Geriatric Services Support (GSS) team, and clinical supervision and training of the GSS team.

Schedule is Monday – Friday, working 8:00am – 5:00pm.

Compensation:

$103,833.60 – $114,483.20 Salary

PACE HEALTH PLAN SPECIALIST (CERES WHITMORE)

The Central Valley PACE Health Plan Specialist supports the team through data analysis, interpretation, reporting, and modeling health plan operations. Supports aspects of monthly and ad hoc reporting and assists in the preparation of pro formas based on historical, comparative, and projection data. Assists to ensure health plan operations, processes, procedures, policies, and compliance efforts are in compliance with regulatory agencies.

Will work in our Ceres Whitmore Clinic. 

Schedule is Monday – Friday, working from 8:00am – 5:00pm

Compensation:

$25.22 – $26.48 an hour

PACE OCCUPATIONAL THERAPIST (MERCED)

The occupational therapist will evaluate and reassess participants upon referral from the Interdisciplinary Team.  Provides and recommends therapeutic exercises. Responsible for the overall occupational therapy program at Central Valley PACE.

Will work in our Central Valley PACE Center in Merced. 

Schedule is Monday – Friday, 8:00am – 5:00pm.

$52.42 – $57.79 an hour ($109,033 – $120,203 annually, 40 hours a week guaranteed)

PACE QUALITY  MANAGER  (MERCED)

Under the supervision of the Director of Center Operations RN, develops, organizes and monitors a Quality Assessment and Performance Improvement Plan (QAPI) for the PACE program. The Quality Manager is responsible for developing the QAPI annual plan, and guiding the implementation of the plan. Ensuring that data is collected from all appropriate sources; and the data is examined and the results are shared with all stakeholders. Oversees the process to evaluate and resolve medical and non-medical grievances and appeals by participants, their family members, or representatives.  Manages the Utilization team and quality specialists.  

Schedule is Monday – Friday, 8:00am – 5:00pm

PACE SPEECH THERAPIST – PART- TIME / PER DIEM (MERCED)

Provide speech-language pathology services to geriatric participants of Central Valley PACE.  These services will be provided in the PACE Center, Nursing and Assisted Living facilities and participant homes. Participates in operational and clinical functions of department, hospital and corporation.  Complies with laws & regulations of appropriate state and federal agencies.  

We can offer either a Part-Time, or Per Diem schedule. Center hours are Monday – Friday, 8am – 5pm (Varied). 

Compensation:

$52.42 – 60.68 an hour
