PACE ADULT GERONTOLOGY NURSE PRACTITIONER (Merced PACE Center)
The PACE Adult-Gerontology Nurse Practitioner (AGNP) provides high-quality, compassionate medical care to participants of the Golden Valley Health Centers (GVHC) PACE program. This position works under the direct supervision of the PACE Medical Director and/or designated primary care physicians and is responsible for assessments, treatment planning, coordination, and delivery of care in the clinic, day center, homes, and residential settings.
Location and Schedule: This position will work at our Central Valley Merced PACE Center, scheduled Monday – Friday, 8:00am-5:00pm. Flexibility to work scheduled evening clinic hours, and when appropriate, act independently as a solo provider of services under telephone communication with a physician supervisor.
Compensation: $164,000 – $194,000 Annual Salary; Sign On Bonus and Relocation Assistance Eligible

EPIC APPLICATION ANALYST I (MODESTO)
The Epic Application Analyst I will be responsible for assigned Epic application design, build, testing, training and support. The Analyst will manage communications, expectations and deliverables between Epic workgroups/councils, internal IT teams, external stakeholders and vendors in support of GVHC Health IT (HIT) systems. This role operates in close coordination with the IT leadership, HIT application and training, IT service desk and IT engineering teams.
This position will work at our Modesto PACE Center. Schedule is Monday – Friday, 8:00am – 5:00pm
Compensation: $34.98 – $37.43 an hour

PACE APPLICATION TRAINER (MODESTO)
The PACE Application Trainer plays a critical role in delivering effective and comprehensive training on the Epic electronic health record (EHR) system and other applications utilized by the PACE program. This position bridges clinical expertise with technical proficiency to ensure seamless adoption, optimal utilization, and continued user competency across all platforms critical to PACE operations.

PACE CLINICAL THERAPIST (MODESTO/MERCED)
Under the supervision of the Medical Director, the Clinical Therapist is responsible for developing and implementing personalized care plans to ensure high-quality, team-based behavioral health services for participants. Key responsibilities include conducting initial assessments, providing clinical interventions, treatment recommendations, and coordinating referrals for participants and their families. This role requires physical presence at a Central Valley PACE center and the flexibility to travel for participant visits as needed. This role collaborates with a multidisciplinary team to provide evaluation, individual, family, and group therapy, as well as case management for older adults with mental health and behavioral needs.
Compensation: $94,000.00 – $98,700.00 Salary
Location: Merced and Modesto Central Valley PACE Centers
Schedule: Monday – Friday, 8:00am – 5:00pm.
PACE GERIATRIC SERVICE ASSISTANT – PER DIEM (MODESTO)
Responsible for providing support and advocacy services to patients of the Central Valley PACE program, including participant screening, planning, coordination of patient services and care, and referral.
Will work at the Central Valley PACE Center in Modesto.
Schedule: Per Diem (as needed). Flexibility to work Monday – Friday, 8:00am -5:00pm, with possibility to be available up to 40 hours if needed.
Compensation:
$26.87 – $28.21 an hour.
PACE GERIATRIC SERVICES SUPERVISOR (MERCED)
Under the direct supervision of the PACE Clinical Manager, the Geriatric Services Supervisor is responsible for coordinating, supervising and evaluating the implementation of psychosocial support services to participants of the Central Valley PACE program including participant screening, assessment, planning, coordination of patient services and care, counseling and referral services carried out by the Geriatric Services Support (GSS) team, and clinical supervision and training of the GSS team.
Schedule is Monday – Friday, working 8:00am – 5:00pm.

PACE HOME CARE COORDINATOR (MODESTO)
Under the direct supervision of the PACE Home Care Admin Manager, the Home Care Coordinator RN is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment and nursing home care, including supervision of Personal Care Assistants (PCA) and Licensed Vocational Nurses (LVN) delivery assistance with participant activities in the center and home environments as assigned.

PACE LICENSED VOCATIONAL NURSE (MODESTO)
Under the direct supervision of the supervising RN, the PACE Licensed Vocational Nurse (LVN) assists in assessing for and delivering clinical skilled services. This position works closely with all PACE departments including the Clinic, Day Health Center, Home Care and supports PACE activities, including on the supervisions of Personal Care Assistants (PCA’s) delivering assistance with Participant activates in the Center and Home environments as assigned.

PACE MEDICAL TRANSPORTER – PER DIEM (MODESTO)
Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.
Location: Central Valley PACE Center in Modesto, Ca.
Schedule: Monday – Sunday – Per Diem (as needed). This position requires availability between the hours of 6:00am-6:00pm. Work hours will vary based on need, with shifts ranging from 2 to 8 hours per day when called upon, not exceeding 15 hours per week.
Compensation: $21.79 – $22.88 an hour
PACE OUTREACH SPECIALIST (MERCED)
The Outreach Specialist plans, organizes, and directs all outreach and marketing efforts. Involved in the intake and enrollment, and coordination with the interdisciplinary team to facilitate new PACE participant enrollment.

PACE PHYSICAL THERAPY ASSISTANT (MODESTO)
The Physical Therapy Assistant (PTA) supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE participants by supporting a physical therapy program under the direction of a physical therapist for PACE patients, helping each achieve independence and safe living abilities.
Will provide Physical Therapy services at Modesto PACE, and may provide support to our Modesto McHenry Specialty site as needed.

PACE QUALITY SPECIALIST (MODESTO)
Under the supervision of the PACE Quality and Compliance Coordinator, the Quality Specialist works to support the annual quality and Compliance (Q&C) work plans and special initiatives by monitoring, gathering, and preparing Q&C data for use by the PACE Interdisciplinary Team and Administration. The Q&C Specialist is a key position that works with all PACE departments to monitor and identify potential Q&C issues, monitor Q&C improvement initiatives, and prepare Q&C summary reports.

PACE TRANSPORTATION AIDE (MODESTO)
Under the general supervision of the PACE Transportation Supervisor, the Transportation Aide supports various aspects of the day center transportation services by assisting with administrative duties such as vehicle maintenance, logs, and invoice reconciliations.

PACE UTILIZATION SPECIALIST LVN (MERCED)
Under the direct supervision of the Utilization Management (UM) Registered Nurse (RN), the PACE Licensed Vocational Nurse (LVN) assists in assessing the need for and delivering clinical skilled services. Provides utilization management functions as a part of the Program of All-Inclusive Care for the Elderly benefits management system. This includes providing utilization review and management for all acute, post-acute, and outpatient services as well as performing the identification, analysis and resolution of resource utilization outliers consistent with established protocols, policies and procedures.
