PACE ADULT GERONTOLOGY NURSE PRACTITIONER (Merced PACE Center)
The PACE Adult-Gerontology Nurse Practitioner (AGNP) provides high-quality, compassionate medical care to participants of the Golden Valley Health Centers (GVHC) PACE program. This position works under the direct supervision of the PACE Medical Director and/or designated primary care physicians and is responsible for assessments, treatment planning, coordination, and delivery of care in the clinic, day center, homes, and residential settings.
Location and Schedule: This position will work at our Central Valley Merced PACE Center, scheduled Monday – Friday, 8:00am-5:00pm. Flexibility to work scheduled evening clinic hours, and when appropriate, act independently as a solo provider of services under telephone communication with a physician supervisor.
Compensation: $164,000 – $194,000 Annual Salary; Sign On Bonus and Relocation Assistance Eligible

EPIC APPLICATION ANALYST I (MODESTO)
The Epic Application Analyst I will be responsible for assigned Epic application design, build, testing, training and support. The Analyst will manage communications, expectations and deliverables between Epic workgroups/councils, internal IT teams, external stakeholders and vendors in support of GVHC Health IT (HIT) systems. This role operates in close coordination with the IT leadership, HIT application and training, IT service desk and IT engineering teams.
This position will work at our Modesto PACE Center. Schedule is Monday – Friday, 8:00am – 5:00pm
Compensation: $34.98 – $37.43 an hour

PACE CLINICAL THERAPIST (MODESTO/MERCED)
Under the supervision of the Medical Director, the Clinical Therapist is responsible for developing and implementing personalized care plans to ensure high-quality, team-based behavioral health services for participants. Key responsibilities include conducting initial assessments, providing clinical interventions, treatment recommendations, and coordinating referrals for participants and their families. This role requires physical presence at a Central Valley PACE center and the flexibility to travel for participant visits as needed. This role collaborates with a multidisciplinary team to provide evaluation, individual, family, and group therapy, as well as case management for older adults with mental health and behavioral needs.
Compensation: $94,000.00 – $100,500.00 Salary
Location: Merced and Modesto Central Valley PACE Centers
Schedule: Monday – Friday, 8:00am – 5:00pm.
PACE GERIATRIC SERVICES SUPERVISOR (MERCED)
Under the direct supervision of the PACE Clinical Manager, the Geriatric Services Supervisor is responsible for coordinating, supervising and evaluating the implementation of psychosocial support services to participants of the Central Valley PACE program including participant screening, assessment, planning, coordination of patient services and care, counseling and referral services carried out by the Geriatric Services Support (GSS) team, and clinical supervision and training of the GSS team.
Schedule is Monday – Friday, working 8:00am – 5:00pm.
Compensation:
$104,332.80 – $114,766.08 Salary
PACE HEALTH PLAN MANAGER (CERES)
Responsible for providing oversight and coordinating the daily operations of health plan objectives including contracts, compliance, credentialing, communication, authorizations/claims, and disputes in a manner that meets established turnaround, quality and production goals, and regulatory standards.

PACE HEALTH PLAN SPECIALIST (CERES)
The Central Valley PACE Health Plan Specialist supports the team through data analysis, interpretation, reporting, and modeling health plan operations. Supports aspects of monthly and ad hoc reporting and assists in the preparation of pro formas based on historical, comparative, and projection data. Assists to ensure health plan operations, processes, procedures, policies, and compliance efforts are in compliance with regulatory agencies.

PACE LICENSED VOCATIONAL NURSE (MODESTO)
Under the direct supervision of the supervising RN, the PACE Licensed Vocational Nurse (LVN) assists in assessing for and delivering clinical skilled services. This position works closely with all PACE departments including the Clinic, Day Health Center, Home Care and supports PACE activities, including on the supervisions of Personal Care Assistants (PCA’s) delivering assistance with Participant activates in the Center and Home environments as assigned.

Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.
Location: Central Valley PACE Center in Modesto, Ca.
Schedule: Monday – Friday. This position requires availability between the hours of 6:00am-6:00pm.
Compensation: $21.79 – $22.88 an hour
PACE MEDICAL TRANSPORTER – PER DIEM (MODESTO)
Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.

Schedule: Monday – Sunday – Per Diem (as needed). This position requires availability between the hours of 6:00am-6:00pm. Work hours will vary based on need, with shifts ranging from 2 to 8 hours per day when called upon, not exceeding 15 hours per week.

PACE ONBOARDING SPECIALIST (MODESTO)
The PACE Onboarding Specialist plays a vital role in supporting the successful onboarding, training, and ongoing competency of all new and existing staff across the Central Valley PACE program. This position is responsible for coordinating and facilitating comprehensive orientation programs, ensuring staff receive timely access to tools, systems, and resources, and maintaining accurate training documentation and compliance records.

PACE OUTREACH AND ENROLLMENT SUPERVISOR (MODESTO)
Assists with the planning, development and coordination of marketing for PACE (Program of All-Inclusive Care for the Elderly); including activities to promote and maximize awareness of PACE within the community. Coordinates daily duties of the outreach and enrollment team.

PACE PHYSICAL THERAPY ASSISTANT (MODESTO)
The Physical Therapy Assistant (PTA) supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE participants by supporting a physical therapy program under the direction of a physical therapist for PACE patients, helping each achieve independence and safe living abilities.
Will provide Physical Therapy services at Modesto PACE, and may provide support to our Modesto McHenry Specialty site as needed.

PACE REFERRAL & MEDICAL RECORDS SUPERVISOR (CERES WHITMORE)
Oversees a team responsible for referral coordination, authorizations, scheduling, medical records, and regulatory compliance within the PACE Health Plan. Ensures smooth health plan operations, manages workflows, supports data-driven decision making, and contributes to audit readiness and compliance with Centers for Medicare & Medicaid Services (CMS), Department of Health Care Services (DHCS), and other regulatory bodies.
