PACE ADULT GERONTOLOGY NURSE PRACTITIONER
The Nurse Practitioner has the responsibility for providing direct and indirect care to the Participants in the PACE Health Clinic and Day Center. Works under the direct supervision of the PACE Medical Director and/or designated primary care physicians as described in the Golden Valley Health Centers PACE Nurse Practitioner’s Supervision Agreement, Golden Valley Health Centers Principles of Practice, and the California Administrative Code, Section 13799.541. The PACE Nurse Practitioner may be required to work scheduled evening clinic hours, and when appropriate, act independently as a solo provider of services under telephone communication with a physician supervisor.

PACE CARE COORDINATOR RN (MODESTO /MERCED)
Under the supervision of the Medical Director and Clinical Supervisor , provides clinic nursing services including assessing participants, participates in the interdisciplinary team and care planning, implements care and case management, evaluates participant care and medical records, and provides clinical leadership
Will work in our Central Valley PACE Centers in Modesto and Merced.
Schedule is Monday – Friday, working 8:00am – 5:00pm.
Compensation: $49.92 – $57.79 an hour
PACE CERTIFIED OCCUPATIONAL THERAPY ASSISTANT (MERCED)
The Certified Occupational Therapy Assistant (COTA) is responsible under the supervision of the Occupation Therapist for implementation of a program of occupational therapy for PACE patients to help them to remain independent and live safely in the community.
Compensation:
$37.25 – $41.07 an hour ($77,480.00 – $85,425.60 annually, 40 hours a week guaranteed)

PACE DAY CENTER RECEPTIONIST (MODESTO)
The Administrative Receptionist provides administrative support to the Director of PACE and leadership team for PACE. Under the direct supervision of the PACE Center Manager, the Administrative Receptionist duties include general clerical, receptionist and project based work, answers all administration calls, type documents, and greets visitors to the PACE Center. The Administrative Receptionist should project a professional and cordial image through personal and phone interaction.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.

PACE DIETARY AIDE (MODESTO)
Under the supervision of the Registered Dietitian(s) & Nutrition Services Specialist, the Dietary Aide assists with daily food and nutrition services for PACE program participants.
Schedule is Monday – Friday, 8:00am – 5:00pm

PACE DIRECTOR OF CENTER OPERATIONS, RN (MERCED)
The Central Valley PACE (Program of All-Inclusive Care for the Elderly) Director of Center Operations (DCO) is responsible for managing all operational aspects for one regional PACE center and associated Alternative Care Site’s (ACS’s) served by multiple PACE Interdisciplinary Teams (IDT’s) to achieve optimum access to services, excellent customer service, quality, cost, and delivery performance in line with the PACE Mission, strategic goals and values, and serves as the liaison between Center, Support Service Management and leadership.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.

PACE GERIATRIC SERVICES SPECIALIST (MODESTO)
Responsible for providing support and advocacy services to patients of the Central Valley PACE program, including participant screening, assessment, planning, coordination of patient services and care, counseling and referral.
Schedule is Monday – Friday, working 8:00am – 5:00pm.
Compensation:
$32.18 – $33.79 an hour
PACE HEALTH PLAN SPECIALIST (CERES WHITMORE)
The Central Valley PACE Health Plan Specialist supports the team through data analysis, interpretation, reporting, and modeling health plan operations. Supports aspects of monthly and ad hoc reporting and assists in the preparation of pro formas based on historical, comparative, and projection data. Assists to ensure health plan operations, processes, procedures, policies, and compliance efforts are in compliance with regulatory agencies.
Will work in our Ceres Whitmore Clinic.

PACE MEDICAL TRANSPORTER (MODESTO)
Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.
Compensation:
$21.79 – $22.88 an hour
PACE MEDICAL TRANSPORTER II (MODESTO)

The Outreach Specialist plans, organizes, and directs all outreach and marketing efforts. Involved in the intake and enrollment, and coordination with the interdisciplinary team to facilitate new PACE participant enrollment.

Will work at our PACE Center in Modesto.
